For referring providers and patients

ADHD Diagnostic Evaluations

Pediatricians, primary care physicians, psychiatrists, therapists, school teams, and college disability offices send patients to us for one reason: the diagnostic testing they cannot run in house, plus documentation that holds up when it reaches a school, a testing agency, or an employer. We evaluate children, teens, and adults, and return a report you and your patient can act on. See our full ADHD evaluation services for background.

What we evaluate

✓
Children
Parent and teacher input alongside direct assessment, with findings written for 504 Plans, IEPs, and CSE review.
✓
Teens
Evaluation that accounts for school demands and, where relevant, documentation for standardized test accommodations.
✓
Adults
Assessment of current function at work and in daily life, with reports suited to college, graduate, licensing exam, and workplace accommodations.

What the evaluati0n covers

A single 60 to 75-minute clinical interview with standardized measures, written up against the criteria bariatric programs and insurers ask for.

✓
Structured clinical interview and history
Symptom onset, course, and impact across home, school, and work, assessed against DSM-5-TR criteria.
✓
Standardized ADHD rating scales
Validated self-report measures, with parent and teacher forms for children and teens.
✓
 Collateral input across settings
Information from the settings where symptoms show up, so impairment is documented in more than one place.
✓
Co-occurring condition screening
Anxiety, mood, sleep, and learning concerns that can mimic or accompany ADHD are screened and noted.
✓
Functional impairment assessment
How symptoms affect performance day-to-day, described in the terms accommodation bodies look for.
✓
Written diagnostic report with recommendations
A clear determination, DSM-5-TR diagnosis where warranted, and recommendations, sent directly to you.

Documentation that holds up where it is submitted

A diagnosis is only useful if the report behind it survives review. Our evaluations are written with the diagnostic clarity and functional detail accommodation bodies require, so your patient is not sent back for more paperwork.

K-12 accommodations: 504 Plans, IEPs, and CSE submissions
College and university: disability and accessibility office documentation
Standardized and licensing exams: SAT, ACT, GRE, GMAT, LSAT, MCAT, bar, and professional boards
Workplace: ADA accommodation requests
