Pre-Surgical Psychological Evaluations

Preparing for surgery often involves more than medical testing alone. In some cases, a psychological evaluation is required or recommended to better understand emotional readiness, expectations, coping, and other factors that may affect the procedure and recovery process.

At You Can Do It, PLLC, we provide pre-surgical psychological evaluations for a range of medical and elective procedures. Our approach is structured, supportive, and clinically thorough, with the goal of helping individuals move forward with greater clarity while also providing the documentation a surgeon, specialist, or program may need.

We offer virtual and in-person evaluations across New York, depending on the service and clinical fit.

What Is a Pre-Surgical Psychological Evaluation?

A pre-surgical psychological evaluation is a structured clinical assessment used to understand emotional, behavioral, and psychological factors that may influence a person’s readiness for surgery or another medical intervention.

These evaluations are often used to:

  • support pre-procedure clearance
  • identify strengths and support needs
  • clarify expectations about treatment and recovery
  • assess emotional readiness and coping
  • provide documentation for surgeons, specialists, or programs

Depending on the procedure, a psychological evaluation may be required, recommended, or pursued independently for added clarity and support.

Who May Benefit From a Pre-Surgical Psychological Evaluation?

A pre-surgical psychological evaluation may be helpful for individuals who:

  • have been referred by a surgeon, pain specialist, neurologist, orthopedic provider, or other medical professional
  • need documentation or psychological clearance before a procedure
  • are being considered for pain-related interventions or bariatric surgery
  • want a better understanding of emotional readiness, expectations, and support needs before moving forward
  • would benefit from a thoughtful assessment as part of medical decision-making

These evaluations can be helpful not only for providers and surgical teams, but also for individuals who want more clarity and confidence before taking the next step.

Types of Pre-Surgical Evaluations We Offer

At YCDI, pre-surgical psychological evaluations may include:

Each evaluation type serves a slightly different purpose, but all are designed to support informed decision-making, readiness, and meaningful next steps.

Spinal Cord Stimulator (SCS) Psychological Evaluations

Spinal cord stimulator psychological evaluations are conducted to assess a patient’s readiness for this advanced pain management procedure. These evaluations look at emotional, behavioral, and cognitive factors that may affect treatment outcomes, including mood symptoms, coping style, pain perception, and expectations for the procedure.

SCS psychological evaluations are typically required prior to approval for the procedure and are often requested by pain management specialists, neurologists, orthopedic surgeons, or neurosurgeons. Reports are typically completed within 2–3 business days.

Chronic Pain Management Psychological Evaluations

Chronic pain management psychological evaluations are conducted to assess the emotional, behavioral, and psychological factors that may influence how an individual experiences pain and how they engage in treatment. These evaluations are often part of interdisciplinary pain care and can help inform a more comprehensive treatment plan.

They may be required or recommended, depending on the type of treatment being considered. In some cases, they are used as part of clearance for more invasive interventions. In other cases, they are used to better understand stress, coping, pain-related beliefs, and support needs that may affect outcomes. Reports are typically completed within 2–3 business days.

Cosmetic Surgery Psychological Evaluations

Cosmetic surgery psychological evaluations are conducted to assess emotional readiness, expectations, and psychological factors that may influence satisfaction with elective procedures. These evaluations may explore areas such as mood symptoms, body image concerns, expectations, insight, and emotional stability.

These evaluations are often recommended rather than universally required, though some surgeons or practices may request them before moving forward. They can also be helpful for individuals who want added reassurance and informed support before an elective procedure. Reports are typically completed within 2–3 business days.

  • readiness determination
  • next-step planning
  • communication with the referring provider

A Clinical Director’s Perspective on Pre-Surgical Readiness

Berenice Rosales-Nadel, LMHC-D, Clinical Director at You Can Do It, PLLC, on how she thinks about these evaluations.

How do you see your role in someone’s surgical process?

I see myself as one voice on a larger team. Your primary doctor, your surgeon or specialist, sometimes a nutritionist, and me, we are all working toward the same thing, which is for you to be ready and to come through your procedure well. The evaluation is not there to stand in your way. It exists because the people guiding your care want to feel confident that you are prepared, and my part is to look honestly at readiness and support so your team has a clear picture before moving ahead.

What does being “ready” for surgery really mean?

Readiness is less about being in a perfect place and more about understanding the change ahead and being honest about where you are with it. A spinal cord stimulator, weight-loss surgery, a chronic pain intervention, each one asks something different of you, and recovery reshapes daily life in real ways. What I am listening for is whether you understand what is coming, what support you have around you, and how you tend to cope when things get hard. When we can name those things together, you walk in more prepared, and your surgical team has what it needs.

Frequently Asked Questions About Pre-Surgical Evaluations in New York

Are pre-surgical psychological evaluations always required?

Not always. Some procedures require a psychological evaluation as part of the surgical approval or clearance process, while others are recommended by a provider or pursued independently for added clarity. Whether one is required depends on the procedure, the surgical program, and in some cases the insurer.

Which surgeries usually require a psychological evaluation?

A psychological evaluation is most often required before bariatric (weight loss) surgery and spinal cord stimulator implantation. Evaluations are also common before other chronic pain interventions and are sometimes recommended before cosmetic procedures. Requirements vary by surgeon and program, so it helps to confirm what yours expects.

Can you fail a pre-surgical psychological evaluation?

A pre-surgical psychological evaluation is not a pass or fail test. Its purpose is to understand your emotional readiness, identify any support that may help your recovery, and give your surgical team useful information. It is uncommon for someone to be found unsuitable for a procedure based on the evaluation alone.

Do you offer pre-surgical evaluations virtually?

Many pre-surgical psychological evaluations are available both virtually and in person across New York and New Jersey, depending on the type of evaluation and clinical fit. Telehealth makes it easier to complete your evaluation on time, even with a busy schedule or distance from our offices.

How long does it take to get the report?

Reports are typically completed within 2 to 3 business days of the evaluation. If your surgery is on a tight timeline, let us know when you schedule so your clearance does not hold up your procedure.

Will you send the report to my surgeon or referring provider?

Yes. With your written authorization, the completed report is sent to your referring surgeon, specialist, or program so it reaches the people who need it for your clearance. You can also receive a copy for your own records.

Do I need a referral to schedule a pre-surgical psychological evaluation?

No referral is required to schedule. Many people are referred by a surgeon, pain specialist, or program, but you are welcome to reach out directly if you know you need an evaluation or want to understand whether one applies to your situation.

Do you accept insurance for pre-surgical psychological evaluations?

For evaluations conducted in New York, we accept Cigna. We also offer self-pay and provide an out-of-network superbill you can submit to your insurer for possible reimbursement across New York and New Jersey. We are glad to confirm your options before you schedule.

How much does a pre-surgical psychological evaluation cost?

Fees depend on the type of evaluation and its complexity. We confirm the cost with you before scheduling so there are no surprises, and we offer self-pay along with an out-of-network superbill for possible reimbursement. Reach out and we will walk you through the options for your specific evaluation.

Are evaluations available in English and Spanish?

Yes. Evaluations are available in both English and Spanish, including the clinical interview, screening, and written report. If you are more comfortable completing your evaluation in Spanish, let us know when you reach out.

What if I am not sure which evaluation I need?

That is completely fine. If you were referred by a provider or received a request from a surgical program, we can review it with you and identify the right type of evaluation before you schedule. You are also welcome to ask questions first if you are still deciding.
